Recently, I had an enlightening experience during a TikTok live session with a holistic doctor. Amidst a fascinating discussion, I asked a question. To make sure I knew he was responding to me, he said "After A V P U" instead of the shorthand "After AVPU," as we would say in the EMS field. (This is my TikTok handle) This made me realize that many people might not be familiar with AVPU, a vital mnemonic in emergency medicine. So, let's dive into what AVPU means and why it's so crucial.
## What is AVPU?
AVPU stands for Alert, Verbal, Painful, and Unresponsive. It's a simple yet effective tool used by medical professionals to assess a patient's level of consciousness quickly. Understanding and using AVPU can be the difference between life and death in emergency situations.
## Breaking Down AVPU
**A - Alert**
The first level of AVPU is Alert. When a patient is alert, they are awake and responsive without any external stimuli. They are aware of their surroundings, can engage in conversation, and can follow commands. In an emergency, finding a patient in an alert state is a positive sign, indicating they are likely not in immediate danger.
**V - Verbal**
The second level is Verbal. If a patient isn't immediately alert, the next step is to see if they respond to verbal stimuli. This means talking to the patient, asking simple questions, or giving commands. If the patient opens their eyes or responds appropriately when spoken to, they are categorized under Verbal.
**P - Painful**
If verbal stimuli don't elicit a response, we move to Painful. This involves applying a mild pain stimulus, such as a sternal rub or a pinch on the ear, to see if the patient responds. The purpose here is to determine if the patient can react to a stronger stimulus, indicating some level of consciousness even if they aren't responding to verbal cues.
**U - Unresponsive**
Finally, if the patient doesn’t respond to any painful stimuli, they are considered Unresponsive. This is the most critical level, indicating a severe decrease in consciousness, requiring immediate and intensive medical intervention.

Recognizing burn injuries is the first step towards effective care. Here's how to distinguish between the different types:
- First-Degree Burns: These are the lightweights of the burn world, typically resulting in redness, pain, and perhaps minor swelling. Think of sunburns or brief encounters with hot surfaces.
- Second-Degree Burns: Stepping up the intensity, second-degree burns penetrate deeper into the skin, often accompanied by blistering, severe pain, and a glossy appearance. They're the souvenirs of a culinary mishap or a run-in with scalding liquids.
- Third-Degree Burns: The heavyweight champions, third-degree burns delve deep, affecting all layers of the skin and potentially underlying tissues. Their appearance ranges from white and leathery to charred and blackened. Industrial accidents or prolonged exposure to flames often lead to these severe injuries.
Treating Minor Burns
When faced with minor burns, prompt action can make all the difference. Here's a simple approach to treatment:
- Cool it Down: Run cool (not cold) water over the burn for 10-15 minutes to soothe the skin and alleviate pain. Avoid ice, as it can further damage the tissue.
- Cover and Protect: Once cooled, cover the burn with a clean, dry dressing to prevent infection and promote healing. Avoid using adhesive bandages on blistered skin.
- Pain Management: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help manage discomfort and inflammation. Depending on the severity you may need to up the ante and offer Fentanyl or Dilaudid. Follow dosage instructions carefully.
Managing Severe Burn Injuries
In cases of severe burns, swift and decisive action is crucial. Here's how to navigate these challenging situations:
- Assess the ABCs: Prioritize airway, breathing, and circulation. If airway compromise is suspected, intervene immediately and seek advanced medical assistance.
- Remove Constricting Clothing: Carefully remove clothing or jewelry from the affected area, taking care not to further injure the skin.
- Cover and Protect: Use a sterile dressing or clean cloth to cover the burn and minimize the risk of infection. Avoid using any creams or ointments on severe burns.
- Monitor for Shock: Severe burns can lead to shock due to fluid loss. Monitor the patient's vital signs closely and be prepared to provide fluid resuscitation if necessary.
Airway Management: Keeping Clear
When burns involve the airway, maintaining clear breathing pathways is paramount. Here's how to approach airway management:
- Assess Airway Patency: Determine if the patient's airway is compromised due to swelling, inhalation injury, or other factors. Look for signs of respiratory distress, such as stridor or difficulty breathing.
- Provide Oxygen: Administer supplemental oxygen to support oxygenation and ventilation. Non-rebreather masks or bag-valve-mask devices are commonly used in these situations.
- Consider Advanced Airway Interventions: If the patient's airway is severely compromised, endotracheal intubation may be necessary to secure the airway and ensure adequate oxygenation.
- Continuous Monitoring: Monitor the patient's airway, breathing, and oxygenation closely, and be prepared to adjust interventions as needed.

To the untrained eye, AVPU might look like just another jumble of letters that medics throw around, but for the initiated, it's the “101” of quickly assessing someone's responsiveness. If you've ever watched an episode of ER (the TV show, not the actual emergency room), you might've seen doctors and nurses shouting "AVPU! Initiate level 3 trauma protocol!" across crowded rooms. Okay, I may have added the dramatic shout, but you get the gist.
Why should EMT students pay special attention to these four letters and what life-saving knowledge do they hold? Let's break it down, shall we?
A - Alert
When we ask if someone is “A” in the AVPU scale, we're not inquiring if they're wide awake and greeting the day with open arms. We're typically checking if the person is alert and oriented to person, place, time, and events.
What it looks like: A person who is alert will respond coherently when spoken to, knows where they are, the current time, and what happened.
How to check: Basically, ask some casual questions: “Can you tell me your name? Where are we right now? What's the date?” Not exactly a pop quiz, but we're not asking for a thesis either.
V - Verbal
Things start to get a bit hairy when we move down the scale to “V.” This is when a person might be groggy, disoriented, or just isn’t making a whole lot of sense when spoken to.
What it looks like: You may need to repeat your questions, and the answers might not be very relevant or coherent. It's like conversing with your uncle after Thanksgiving dinner, but make it a little less about alien conspiracies.
How to check: Don’t get too creative here; just ask some direct, but simple, questions.
P - Painful
Reaching the "P" stage is when things get real. If someone is only responding to painful stimuli, that's a significant red flag.
What it looks like: Responding only to pain means the individual is unresponsive to vocal or verbal stimuli but will react to pain – or even the mention of a high school P.E. class. (not really)
How to check: A sternal rub (yes, it's still a thing) but you only need to do it once, or a light pinch on the shoulder should yield some type of response.
U - Unresponsive
Finally, we come to “U,” a place where we fear to tread, but find ourselves all too often in the profession. If a person is unresponsive, especially after applying painful stimuli, it's a serious situation.
What it looks like: Unresponsive individuals are, as the name suggests, not responding to pain or any other form of stimulus. Bilateral pupils not responding to light would be of particular concern.
How to check: The good old tap-and-shout, followed by a sharp “Hey! Are you okay?” If there's no response, following it up with one sternal rub may stir a sleeping response.
